APPLYING FOR ADMISSION
Teamwork Admissions Staff look forward to the opportunity to guide you through our admissions process. GENERAL ADMISSION REQUIREMENTS
1. All applicants must:
a) Provide proof of government approved Photo ID, Driver’s License or Birth Certificate
b) Read, agree, sign, and receive a copy of the enrollment application and school handbook.
c) Must be able to speak English fluently
d) Must be free of communicable disease
2. The ability to read and write English must be demonstrated by completing all program applications in person.
3. Acceptable documents for the Nurse Aide Program are:
a) Proof of high school completion,GED or equivalent test given on site.
b) Copy of current Driver’s License or US government issued Photo identification
c) Copy of Social Security Card (SSN) or Tax Identification Number (TaxID)
d) Copy of TB Skin or X-ray test (must be negative)
e) The student cannot be listed as unemployable on the Employee Misconduct Registry (EMR) and cannot have been convicted of a criminal offence as listed in Texas Health and Safety Code 250.006
Class curriculum and class scheduled hours must be successfully completed with a minimal grade of “C” in each theory course and a “P” in each clinical course/ externship before the student is able to receive a certificate of program completion from TEAMWORK CAREER INSTITUTE. All test, quizzes, examination, exit test must be passed successfully and finalized as well as acceptable clinical competency evaluations completed in order to be considered for receipt of a certificate. Graduation ceremony is not an indication of successful completion of programs.
